Honorary Life Membership.
If you are planning on honoring one of your PTA members with an Honorary Life Membership at the P&P Luncheon and have already sent the form to the State office and did not fill it out directing the State office to send the Certificate and Pin to Annette Worthen, please contact her at annetteworthen@gmail.com and let her know to add the recipient to her list. If you have not sent the form to the State office yet, please fill it out, including the section on where to send it. Have the Certificate and Pin sent directly to Annette Worthen, (call her or email for the address). Honorary Life Membership forms are located at Floridapta.org in the restricted area or call 1-800-373-5782.
